The affair had ended, she said, and both of them were trying to mend their personal lives.
She faces three years of unsupervised probation and must reimburse the city $11,000 for what prosecutors called "unlawful expenditures".
Barry pleaded guilty to a charge of felony theft of more than $10,000.
Reports say Forrest earned more than $170,000 in overtime pay while serving Barry. During that time, their romantic affiliations allegedly occurred on the clock and therefore, resulted in the theft of property from the Metro Nashville city funds amounted to $10,000 spent in the relationship.
Barry was indicted just days after a judge in Nashville issued a search warrant based on a request from the Tennessee Bureau of Investigation seeking to take possession of Barry's cell phone.
"The allegations of photos taken of myself are very troubling and infuriating if true", Barry said in a statement after the documents were released. She was facing multiple investigations regarding a suspicious amount of overtime Forrest drew while traveling with her on multiple trips overseas in her role as mayor.

But Barry - who was elected as the city's first female mayor in September 2015 - refused to quit, insisting that no professional misconduct had taken place.
Barry admitted to the affair on January 31 but did not step down as mayor.
Before ascending to the mayorship, Barry was as an ethics and compliance officer at Premier Ethics and Compliance.
Prior to the affair, Barry was considered a rising progressive star in Nashville where she had approval ratings around 70 percent.
