They moved seven points clear of second-placed Tottenham, and all but consigned Middlesbrough to relegation.
The Stamford Bridge side will clinch the title with two games to go if they beat West Brom on Friday, but Conte is adamant it was by no means an easy season for Chelsea.
Asked what he had said to his players following the Middlesbrough win, Conte replied: "The message was now we need to take another step".
"It's been a different year for me, I'm used to playing every year at every club I've been at but I've matured a lot", Fabregas told Sky Sports.
"I saw, this season, that he put himself in this team in every moment - if he played or if he didn't play".
And Courtois says it would send a message to the critics who slammed the team past year.
Midfielder Nemanja Matic, who scored his first goal at Stamford Bridge against Boro, reckons Chelsea's ability to respond from setbacks has been key.
A win at the Hawthorns on Friday against West Brom will guarantee Chelsea this season's Premier League title. For us it was very important to play and to win today.
Warriors close out sweep of Jazz in dominant fashion
The third quarter saw a brief spark by the Jazz and they cut the lead to 6 points, 66-60 in the first four minutes of the period. Durant wasn't almost as productive as Game 4 , when he popped 38 points, but his 18 points in this one were pretty timely.
"Over the last two seasons, teams who have not big possession have won the league", he said.
Courtois added: "We hope to do it at the first attempt".
"I have to say thanks to the club, players and staff to work very hard to change this".
His nine Premier League appearances before departing in December for Chinese side Shanghai SIPG are enough to warrant a winners medal.
"We're leaving it late but we've still got three games left to get 50 points, there's still a desire in the group to do that and we want to finish strong". We decide our future.
"It wasn't like we felt we had to score because the title was at stake".
Conte admitted that it has been a very hard season for Chelsea. Bad night, a really bad season, the lowest point of my career by a long way and everyone else in the dressing room. "Of course we are happy we are so close but we have to stay concentrated to the end".
"That's helped us this time, having the experience of that".
